Thank you for choosing The Beloit Mattress Company. Use this form to register a new Beloit mattress or eligible Beloit-branded bedding accessory. Registration helps us connect your product with your purchase information so our team can provide faster support if you ever need warranty service, product assistance, or care information.
Mattress & Accessory Warranty Registration
Registering a Mattress?
Before you begin, locate the law label or product tag sewn to your mattress. You will need to enter the mattress serial number exactly as it appears on the tag.
In the example shown, both the model and serial number are circled in red at the bottom of the tag, near the “Date of Delivery” line. Please enter your serial number carefully, including any letters, numbers, and dashes.
Do not remove the mattress tag. Bedding accessories do not have serial numbers and can be registered without one.

Why Register Your Product?
Product registration creates a convenient record of your mattress or bedding accessory and helps us support you more efficiently.
- Faster Product Identification — Helps us identify your mattress or accessory when you contact us.
- Easier Warranty Support — Connects your product, serial number, and purchase details in one record.
- Care and Maintenance Information — Helps us provide guidance appropriate for your product.
- Registration Confirmation — Gives you a record that your registration was submitted.
Product registration does not extend, modify, or replace the applicable written warranty. Please retain your original purchase receipt and do not remove the law label or tag from your mattress. Warranty eligibility remains subject to The Beloit Mattress Company’s published warranty terms.
This form registers a product. It does not submit a warranty claim. To report a possible warranty issue, please contact our factory.
